Position Overview: As an Advanced Practice Provider (APP) at Dimer Health, you will play a critical role in delivering virtual predictive, proactive care. This role is open to both Physician Assistants (PAs) and Nurse Practitioners (NPs). You will provide high-quality, patient-centered care through telehealth platforms while working alongside a multidisciplinary team to monitor patient progress, address concerns, and optimize care to ensure smooth transitions from hospital to home.
KEY RESPONSIBILITIES:
- Virtual Predictive, Proactive Patient Care
- Conduct telehealth consultations with medical and surgical patients throughout varying stages of their health journey.
- Review patient records, lab results, and discharge summaries to risk stratify and develop personalized care plans.
- Identify early risk factors and warning signs of complication and proactively intervene to improve outcomes.
- Collaborate with physicians, nurses, case managers, and other healthcare professionals to ensure cohesive care for patients.
- Assist in developing and modifying individualized care plans for patients based on their progress and needs.
- Educate patients on self-management strategies, medication adherence, and lifestyle changes to promote long-term health.
- Respond to asynchronous patient communications and ensure timely interventions to prevent complications.
- Provide emotional support to patients and families.
- Empower patients to take an active role in their recovery, fostering independence and confidence in managing their health.
- Maintain accurate, detailed, and timely records of patient interactions, care plans, and progress in the company’s electronic health record system.

- Collaboration with Technology and Innovation Teams
- Work closely with Dimer Health’s team to optimize the digital tools used for patient monitoring, reporting, and communication.
QUALIFICATIONS:
- Education & Licensure
- Degree in a relevant clinical field (Physician Assistant Studies or Nurse Practitioner program) from an accredited program.
- Board certification as a Physician Assistant (PA-C) or Nurse Practitioner (NP).
- Active state licensure (NY or NJ).
- Experience
- Minimum of 2 years of clinical experience, with a preference for experience in emergency medicine, post-acute, transitional, or hospital-based care.
- Prior experience in telehealth or virtual care is a plus.
