The “Transitionist®” is a new clinician role designed to own the vulnerable window of time after a hospital discharge or medical event. By combining human oversight with technology, transitionists close care gaps, reduce readmissions, and ensure no patient feels abandoned once they leave the hospital.

As a physician assistant and health-tech founder, I’ve seen firsthand how vulnerable patients can be the moment they leave the hospital. This article is a call to action: we need new roles, like the Transitionist®, to own those first hours and days after discharge, powered by technology but rooted in human connection. My hope is that this piece sparks dialogue about how we can reimagine recovery and build systems that keep patients safe, supported, and out of the ER.
