It marks the validation of a simple idea we believed from day one: what happens after a patient goes home from the hospital is one of the most important, and most overlooked, periods in healthcare.

Instacart didn’t invent groceries - it changed how people get groceries.

Each transformed the user experience - making it instantaneous, personalized, and available on demand. This is how people experience everything now.

Healthcare, especially the discharge and post-discharge experience, has failed to keep up.

Healthcare is highly coordinated inside the hospital, the OR, the clinic. But once a patient goes home, the system becomes fragmented, reactive, and hard to navigate. Patients and families are left managing medications, symptoms, appointments, complications, and uncertainty, often alone. Providers are left hoping nothing goes wrong.

Too often, patients end up back in the emergency room with complications that could have been prevented with the right support at home.

The discharge experience needed to evolve. So we built something new.

What We Built

We built Dimer Health to support patients where recovery actually happens: at home. We designed a new model of post-discharge care that combines clinical care, technology, and artificial intelligence to support patients continuously after discharge.

That foundation includes:

• A clinic of specialized providers dedicated entirely to the post-discharge period, we call them Transitionitsts®
• Transitional care protocols designed specifically for recovery
• Cura, Our AI powered platform to provide personalized, continuous care at scale
• AiME, our personalized AI healthcare companion
• A hybrid care model where AI and clinicians work together to support patients continuously after discharge

Our goal is simple: make sure no patient feels alone after they go home.

How Far We’ve Come

Since founding Dimer Health in 2023, we have:

• 5,883 episodes of care
• More than 10,000 visits
• Partnered with 29 health systems, providers, and payors
• Patients who utilize Dimer as part of their care team are up to 45% less likely to require a hospital admission after discharge
• Achieved 79.63 patient satisfaction / NPS
• Built and launched AiME and Cura
• 500 patient chats with AiME
• Built a new Transitional Care clinical model
• Proven that a continuous, hybrid care model improves outcomes and patient experience

This progress is what made this Series A possible.

What this Series A Allows Us to Do

This funding allows us to expand into new markets, grow our clinical teams, continue building our AI and technology platform, partner with more health systems, providers, and payors, and reach more patients during the most vulnerable period of recovery.

This round is about scaling a company, and more importantly, it’s about scaling a new model of care.
