Back to Your Doctor

Dimer Health is not affiliated with a specific health system. We work in conjunction with, and act as an extension of, your physician to continue your medical surveillance after discharge.
We communicate with them in real-time if any urgent issues arise, and we collaborate to ensure your entire care team is connected and informed about your recovery journey.
You will be seen by one of our licensed medical providers for a virtual physical exam. During this visit, we will have a complete discussion of your most recent healthcare encounter and a review of any chronic conditions.
The provider will also thoroughly review your discharge instructions, so you have a clear understanding of your current health journey. We connect all the components of your care to ensure that your complete health is managed seamlessly. We also encourage open dialogue so that all questions can be answered.
You are being referred to us because you are now going through a transition of care. We work collaboratively with your healthcare team to ensure all aspects of your medical and social needs are factored into your Patient Care Plan to promote better outcomes.
If you do not have a physician or require a specialist, we will help you to coordinate care with the appropriate providers.
Dimer Health will bill your insurance company directly for the services you receive from us. You will be responsible for your co-pay. We follow the same process with regards to insurance and billing as your other doctors’ visits.
