Bergen New Bridge Medical Center is working with Dimer Health to deploy an AI tool within its ADT system that screens patients before discharge. Through AiME, at-risk Medicaid and uninsured patients are connected to Transitionists®, who coordinate follow-up care and access to resources, with patient satisfaction reaching 4.85/5 and time to next appointment down to 2.5 days.
