DispatchHealth Co-founder Kevin Riddleberger Joins Dimer Health as Advisor to Advance Post-Discharge Care Transformation

Short Hills, NJ – September 23, 2025 – Dimer Health, a leader in transitional care medicine, today announced that Kevin Riddleberger, PA-C, MBA, has joined as an Advisor to the company’s leadership team. Kevin is a healthcare entrepreneur and co-founder of DispatchHealth, where he scaled the company from a single-market pilot to the nation’s largest high-acuity, in-home care network across 50 markets and 30 states.
As health systems continue to face mounting readmission penalties, workforce shortages, and unsustainable operating margins, Dimer Health is doubling down on its mission to transform recovery after discharge. The company combines 24/7 physician-led virtual care with proprietary AI that identifies risk factors and surfaces actionable insights, preventing complications before they escalate into costly ER visits or hospital readmissions.
“One in five patients are readmitted within 30 days of discharge, and many of those readmissions are preventable,” said Caroline Hodge, PA-C, Co-Founder and CEO of Dimer Health. “Our model combines human touch with AI-driven clinical monitoring to intervene early, keep patients safe at home, and relieve the crushing burden on hospitals and provider teams. Kevin’s expertise scaling innovative care models will help us expand this impact nationally.”
Kevin brings more than two decades of combined clinical, executive, and entrepreneurial experience to Dimer Health. At DispatchHealth, he architected an omni-channel care platform that saved health systems and payers over $1.5 billion in documented medical costs while maintaining a 95+ Net Promoter Score (NPS). His background includes leading clinical strategy at iTriage and advising multiple digital health startups focused on value-based care, senior care orchestration, and AI-enabled navigation.
“Hospitals and health systems are under pressure like never before, with tighter margins, exhausted staff, and patients falling through the cracks,” said Kevin Riddleberger. “Dimer’s combination of 24/7 physician-led coverage, patient-specific AI insights, and proactive engagement is exactly the solution this moment calls for. I’m excited to help scale a model that keeps patients out of the hospital, improves provider satisfaction, and reduces costs for the entire system.”
Kevin will serve alongside fellow Advisor Alan Leiber, a leader in health system innovation who spent nearly 40 years as a healthcare executive, including 18 years as President of Overlook Medical Center and Vice President of Atlantic Health System. Together, Kevin and Alan will help guide Dimer Health’s strategy, partnerships, and growth as the company continues its mission to transform recovery after discharge.
